Complete Your New Patient Forms
Required before your child’s first Fleet visit
Please complete the Policy Agreement and Payment Preference Form before services begin.
Fleet Patient Services will send the form link directly to the parent or guardian phone number via text.
How Fleet Care Works
Therapy delivered right at your child’s daycare or school.
We Coordinate With the Daycare or School
Our team works with your child’s daycare or school to arrange an appropriate treatment space and visit time.
Your Therapist Comes to Your Child
Your child receives therapy during their regular daycare or school day. Parents do not need to attend each treatment session, but are always welcome.
Our Fleet team will provide frequent updates about your child’s care and is available to answer questions along the way.
You Stay Connected
Review visit notes and progress through the patient portal and contact the Fleet team whenever questions arise.

Important Schedule Changes
Please notify the Fleet team as soon as possible if:
Your child will be absent from daycare or school.
Your child will not be available for therapy.
Your child changes daycares or schools.
Your child’s daycare or school schedule changes.
Your child will be unavailable for an extended period.

Payments Are Processed Weekly
All patient-responsibility payments are processed automatically each week using the card saved on file.
Card on File Required
A card on file is required before your child’s first therapy session. The Patient Services team will assist with getting this set up.
Stay Current on Your Balance
An outstanding balance can become a barrier to your child’s care. Please contact our Billing Department early if you are experiencing difficulty with your balance.

Keeping Your Child’s Care Team Informed
This information will be added to your child’s chart so it is available to the appropriate members of their care team.

Attendance & Scheduling
Consistent attendance helps your child get the most from their therapy.
Consistent Attendance Matters
Consistent attendance is one of the most important factors in your child’s progress.
If your child will be absent from daycare or school or will otherwise be unavailable for therapy, please call or text 479-439-1023 as soon as possible. We will work diligently to reschedule the appointment for the same week, if possible.
Maintaining Your Child’s Therapy Time
To maintain a permanent therapy schedule, patients must meet both attendance requirements below.
Excused Absences
Excused absences may include:
- Doctor’s appointment with at least 24-hour notice
- Illness with at least three-hour notice
- Hospital visit
- Family emergency
- Preapproved vacation of no more than two weeks
- Daycare or school closure
- Other absences approved by management
Unexcused Absences
Unexcused absences may include:
- Failure to notify Fleet that your child will be absent
- Your child being unavailable when the therapist arrives
- Forgetting the scheduled visit
- A no-show
- Less than three-hour notice
- Canceling after the session is scheduled to begin
Extended Absences
Contact our Fleet scheduling team for approval before an absence of two or more weeks.
A child who will be unavailable for four or more weeks may be removed from the permanent schedule and placed on the waitlist. We will do our very best to resume consistent therapy times as soon as the patient is able.
Possible Fleet Schedule Changes
Because Fleet therapy takes place in the community, schedules may occasionally change due to circumstances outside of the normal therapy schedule.

When Should My Child Stay Home?
Please keep your child home from daycare or school—and therefore unavailable for Fleet therapy—if they have any of the symptoms below.
If your child’s daycare or school has stricter guidelines, please follow those guidelines.
Fever
A temperature of 100.4°F or higher. Your child must be fever-free for 24 hours without fever-reducing medication before returning.
Vomiting
Two or more episodes within 24 hours.
Diarrhea
Three or more watery stools within 24 hours.
Pink Eye
Red or pink eyes with discharge.
Severe Coughing
Coughing episodes that cause gagging, vomiting, or difficulty breathing.
Sore Throat
A sore throat accompanied by fever or swollen glands. A sore throat alone does not necessarily require your child to remain home.
Unexplained Rash
A rash that is not associated with heat, diapering, or a known allergy.
Contagious Conditions
This includes ringworm, impetigo, lice, and scabies. Your child may return after receiving appropriate treatment or clearance according to FTC and daycare or school requirements.
Multiple Mouth Sores With Drooling
Your child should remain home unless a physician confirms that the condition is not infectious.
